On 31.3.2023, Government of Kerala amended KMMC Rules, 2015 and some of the forms were changed. The list of such forms are given below.
കേരള മൈനർ മിനറൽ കൺസെഷൻ റൂൾസ്, 2015-ൽ 31.3.2023-ന് ഭേദഗതികൾ വരുത്തുകയും നിലവിലുള്ള ചില ഫോമുകൾ മാറ്റുകയുണ്ടായി പതിവായി ഉപയോഗിക്കുന്ന ഭേദഗതി ചെയ്ത ഫോമുകളുടെ ലിസ്റ്റ് ചുവടെ ചേര്‍ത്തിരിക്കുന്നു.
1. ഫോറം എ: ഖനന പെര്‍മിറ്റിനായുള്ള അപേക്ഷ ഫോറം / Form A: Application for quarrying permit
2. ഫോറം ബി: ഖനന പാട്ടത്തിനായുള്ള അപേക്ഷ ഫോറം / Form B: Application for quarrying lease
3. ഫോറം എച്ച് : ലീസ് ഡീഡ് / Form H: Lease deed (Download word file)
4. ഫോറം എന്‍: സാധാരണ മണ്ണിന്റെഖനന പെര്‍മിറ്റ്‌ / Form N: Quarrying Permit for ordinary earth
5. ഫോറം എം: ഖനന പെര്‍മിറ്റ്‌ / Form M: Quarrying permit
6. ഫോറം എസ്: കെട്ടിടം നിര്‍മാണവുമായി ബന്ധപെട്ട് സാധാരണ മണ്ണിനു നല്‍കുന്ന മൂവ്മെന്റ് പെര്‍മിറ്റ്‌ /Form S: Movement Permit for ordinary earth issued in connection with construction of building (Download word file)
7. ഫോറം ടി: കെട്ടിടം നിര്‍മാണവുമായി ബന്ധപെട്ട് സാധാരണ മണ്ണിനു പാസ്‌ ലഭിക്കുന്നതിനു നല്‍കുന്ന സത്യവാങ്മൂലം / Form T: Affidavit to be filed for obtaining movement permit for ordinary earth in connection with construction of building (Download word file)
8. ഫോറം യു: കെട്ടിടം നിര്‍മാണവുമായി ബന്ധപെട്ട് സാധാരണ മണ്ണിനു പാസ്‌ നല്‍കിയതുമായി ബന്ധപെട്ട് പെനാല്‍റ്റി തുക ഈടാക്കിയത് സംബന്ധിച്ച് റിപ്പോര്‍ട്ട്‌ ചെയ്യുന്നതിനുള്ള ഫോറം / Form U: For reporting penalty collected from offenders in connection with grant of movement permit for ordinary earth (Download word file)
